What to include
- The facility's name and license number (shown on its page), if it's about a facility.
- The address (URL) of the page you're looking at.
- What looks wrong, and what you expected to see.
Disputes about a citation or report
We can't change the State's records. Every citation, complaint and report here is copied from the Community Care Licensing Division (CCLD), so a dispute about a citation, a finding or a report itself has to go to CCLD; each facility page lists its licensing office and phone number. Once CCLD corrects its record, the change shows up here after the next refresh. See "Corrections and disputes" in the Terms of Use.

Privacy
We use your email only to reply to you. What else the site collects (very little) is in our Privacy Policy.
